The No Yes-Man Rule
Outputs self-report their confidence and what information is missing. Below 0.4 on a 0-to-1 scale, or on missing information, execution is blocked. Unsure means stop, not bluff.
Risk-Scored Before It Runs
Every planned output is scored from 0 to 1. Low passes. Medium passes with warnings. High is corrected and retried.

At most 3 correction rounds, then a person takes over with the full history. Alerts arrive by email or webhook the moment they happen.
